29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their bodies in the danger zone during operating cycles: On or about 02/13/2013, in the Machine Shop, the employer failed to ensure that the point of operation/cutting element of an Acer E-mill vertical milling machine (Model EVS-3VKH, serial # N9110105) was equipped with a guard to prevent any part of the operator's body from entering the danger zone while drilling vent holes and clips (steel).
