The employer is failing to protect employees from potential fall hazards associated with unprotect work platforms.  This was most recently evident during the inspection of the basement area of the boot pit area of the grain handling facility located at 1407 W. 4th Ave, Holdrege, NE 68949, where the employer has employees walking on the unprotected work platforms more than 14 feet above the ground, to observe the off-loading of grain delivery trucks.

29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other expose(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR DEPARTMENTS AND THE NUMBER OF EMPLOYEES EXPOSED) (DESCRIBE CONDITIONS INCLUDING EXPOSURE LEVELS, SHIFT, AND DATE)  The employer is failing to protect employees from potential combustible dust hazards associated with a grain handling company operating without an effective written housekeeping program.  This was most recently evident during the inspection of the basement area of the grain handling facility located at 1407 W. 4th Ave, Holdrege, NE 68949, where the employer has employees handling/moving the grain without implementing an effective housekeeping program which was visibly demonstrated with fugitive grain dust build up in excess of 2 inches.  Instances include but are not limited to:              i. Grain pushed by brooms to the ends of the basement tunnel where the drag conveyor runs and left for more than a week.             ii.  Fugitive dust build up over an inch on top of the drag conveyor duct system within 30 feet of the leg.            iii.  Fugitive dust build up over an inch on the floor of the head house.

This was most recently evident during the inspection of the basement area of the grain handling facility located at 1407 W. 4th Ave, Holdrege, NE 68949, where the employer has employees handling/moving the grain from boot pits and bins, via legs and drag conveyors throughout the facility and has allowed for combustible fugitive grain dust build up in excess of 2 inches.
