29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instance 1: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the rotating element/chuck of the 12" lathe used to repair steel parts was equipped with a chuck guard; manufactured by Yamazaki Machine Works, Ltd. Instance 2: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the rotating element/chuck of the 20" lathe used to repair steel parts was equipped with a chuck guard; manufactured by LeBlond Ltd. Instance 3: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the rotating element/chuck of the 16" lathe used to repair steel parts was equipped with a chuck guard; manufactured by ACRA Machinery. Instance 4: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the rotating element/chuck of the 14" lathe used to repair steel parts was equipped with a chuck guard; manufactured by Kingston Machine Tool.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: Instance 1: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the points of operation of the vertical milling machines used to repair steel parts were equipped with a guard; manufactured by Republic Lagun Machine Tool Company. Instance 2: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the point of operation of the horizontal milling machine used to repair steel parts was equipped with a guard. Instance 3: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the point of operation/drill bit of the Wilton drill press was equipped with a guard to protect the operator while drilling holes in carbon and stainless steel. Instance 4: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the point of operation/drill bit of the Tominaga Machinery Drill Press was equipped with a guard to protect the operator while drilling holes in carbon and stainless steel. Instance 5: On or about 02/14/2013, in the Machine Shop, the employer failed to ensure that the point of operation/drill bit of the Republic Lagun Machine Tool Co. drill press was equipped with a guard to protect the operator while drilling holes in carbon and stainless steel. Instance 6: On or about 02/14/2013, in the Machine Shop, the employer did not ensure that the point of operation of the Dake Machine Tools hydraulic press was equipped with a guard to protect the operator while pressing steel.
