29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided for immediate emergency use. On or about February 14, 2013 and times prior thereto, an eye wash station facility was located in an area where mud, high pressure hoses, and uneven surfaces exist. These conditions exposed employees to slip and trip safety hazards, making the eyewash station not available for immediate emergency use.

On or about February 14, 2013 and times prior thereto a microwave oven being used in Rig # 221 at the dog's house was not provided with a permanent, continuous and effective path to ground. This condition exposed employees to electrical safety hazards.
