29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    (a) Lower deck, Outside sewage lab:  On or about 11 February 2013 and times prior thereto, the floor decking, had an opening that measured approximately 18 by 10 inches.  Hole depth was approximately 14 inches.  This condition exposed employees to tripping hazard.       (b) Lower deck, Inside sewage lab:  On or about 11 February 2013 and times prior thereto, the floor decking, inside the sewage lab, had an opening that measured approximately 5 by 7 inches.  Hole depth was approximately 14 inches.  This condition exposed employees to tripping hazard.

29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards.  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    Above Flash Freezer:  On or about 11 February 2013 and times prior thereto, the deck on top of the flash freezer did not have a railing installed.  This condition exposed employees to a 9 foot-7 inch fall hazard.      Icicle Seafoods was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307507640, Citation 01, Item Number 01(a), issued 03/29/2011, with respect to a workplace located on the P/V Bering Star.
