29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:    (a) Jobsite, 14800 Route 30, North Huntingdon, PA:  On or about 02/15/13 the 16 ft. platform extended over its support on the ladder approximately 2 feet and was not installed to prevent the cantilevered portion from tipping.  The platform was not secured or tied to the ladder to prevent movement or tipping nor was a guardrail installed to prevent employees from accessing the portion beyond the support.  This condition presented a potential fall hazard to the employees of approximately 18 feet to the ground.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    (a) Jobsite, 14800 Route 30, North Huntingdon, PA:  On or about 02/15/13 a safe means of access was not provided to access the scaffold platform that was approximately 18 feet above the ground.  The employees did not have an independent ladder to use to access the platform and climbed the ladders used to support the scaffold platform(s) to get to the working level.

29 CFR 1926.452(k)(5): Scaffold platforms shall not be bridged one to another:    (a) Jobsite, 14800 Route 30, North Huntingdon, PA:  On or about 02/15/13 the 16 ft. and 20 ft. platforms on the ladder jack scaffold were bridged one to another and were resting on the same ladder.   The platforms were not tied or secured to the ladder or each other to prevent movement.  The employees were working from the platform(s) to install shingles on the mansard roof.  The potential fall hazard to the employees working on the platforms was approximately 18 feet to the ground.
