29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    On or about February 14, 2013, two trailers where the directional drillers were staying were not grounded properly; the grounding rods/pipe electrodes were installed only approximately 6 inches in the ground.  Employees were potentially exposed to electric shock and/or burn.
