On or about February 20, 2013, an employee who operated a leaf blower was exposed to being struck by flying debris.  The employee used the blower to clear the concrete surface of debris such as pebbles that had fallen off the roof during the repair work.

On or about February 20, 2013, employees who were repairing the roof for a portico entrance of a commercial building were exposed to a fall hazard.  The fall distance from the roof to the concrete surface below was approximately 13 feet.
