29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about February 20, 2013, an employee who operated a leaf blower was exposed to being struck by flying debris.  The employee used the blower to clear the concrete surface of debris such as pebbles that had fallen off the roof during the repair work.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about February 20, 2013, employees who were repairing the roof for a portico entrance of a commercial building were exposed to a fall hazard.  The fall distance from the roof to the concrete surface below was approximately 13 feet.
