29 CFR 1910.134(f)(6): Qualitative fit tests were performed on negative pressure air-purifying respirators when the required fit factor was greater than 100:     a)  for the 2 employees replacing the baffle filters of the ventilation system located at 12000 US Hwy 19 N, Clearwater (FL), required to wear a MSA Advantage full face respirator with GME-P100 cartridges, as observed on or about 02/28/2013.  In that employee 1 was exposed to 4600 ug/m3 of Lead over a 21 minute time period.  This is equivalent to an 8-hour TWA exposure of 201 ug/m3, which represent 4.02 times the OSHA-PEL of 50 ug/m3.  Employee 2 was exposed to 6800 ug/m3 of Lead over a 23 minute time period.  This is equivalent to an 8-hour TWA exposure of 326 ug/m3, which represent 6.52 times the OSHA-PEL of 50 ug/m3.

29 CFR 1910.1025(c)(1): The employer shall assure that no employee is exposed to lead at concentrations greater than fifty micrograms per cubic meter of air (50 ug/m(3)) averaged over an 8-hour period.      a) On or about 02/28/13, the employee 1 changing the baffle filter of the ventilation system was exposed to lead-in-air at a 8-hour time weighted average of 201 ug/m3 which is approximately 4 times the adjusted permissible exposure limit of 50 ug/m3 for an 8-hour work shift.  The exposure level was derived from a sample collected on February 28, 2013 during  21 minutes of sampling period.  The exposure calculation includes a zero exposure for the 459 minutes not sampled during the 8hr work shift.     b) On or about 02/28/13, the employee 2 changing the baffle filter of the ventilation system was exposed to lead-in-air at a 8-hour time weighted average of 326 ug/m3 which is approximately 6.8 times the adjusted permissible exposure limit of 50 ug/m3 for an 8-hour work shift.  The exposure level was derived from a sample collected on February 28, 2013 during 23 minutes of sampling period.  The exposure calculation includes a zero exposure for the 457 minutes not sampled during the 8hr work shift.

29 CFR 1910.1025(e)(5): Administrative controls were used as a means of reducing employees time weighted average exposures to lead and the employer did not establish and implement a job rotation schedule which included the required elements:    a) At facility located at 12000 US Hwy 19. N in Clearwater, FL.  Employer did not implement an adequate job rotation schedule that includes the following required items:     1. Name or identification number of each affected employee;  2. Duration and exposure levels at each job or work station where each affected employee is located;  3. Any other information which may be useful in assessing the reliability of administrative controls to reduce exposure to lead.    Employee 1 was exposed to 4600 ug/m3 of Lead over a 21 time period.  This is equivalent to an 8-hour TWA exposure of 201 ug/m3, which represent 4.02% of the OSHA-PEL of 50 ug/m3.  Employee 2 was exposed to 6800 ug/m3 of Lead over a 23 minute time period.  This is equivalent to an 8-hour TWA exposure of 326 ug/m3, which represent 6.52% of the OSHA-PEL of 50 ug/m3.

Facility located at 12000 US Hwy 19 N. in Clearwater Florida:  Employer did not maintain surfaces as free of Lead as practicable.  Employees are exposed to Lead accumulations on work surfaces.    a) On or about 02/20/13, Lead accumulations of 4.1935 ug were found on an area of 100 cm2 over the range master desk using the wipe sampling method.    b) On or about 02/20/13, Lead accumulations of 3.7160 ug were found on an area of 100 cm2 around the locker room door handle using the wipe sampling method.
