Broadway, Denver, CO 80204: On 2/19/13, the employer did not ensure that a trench, which measured approximately 4 ft. x 9 ft. x 6.3 ft. deep, was equipped with a stairway, ladder, or ramp. This condition exposed an employee to struck-by hazards.

29 CFR 1926.652(a)(1)(ii): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). (a) GDL Industries, Inc. dba Mr. Rooter of Northern Colorado @ 601 S. Broadway, Denver, CO 80204: On 2/19/13, the employer did not ensure that an employee was protected from cave-in hazards. An employee conducted domestic water line repair while inside a trench. The trench measured approximately 4 ft. x 9 ft. by 6.3 ft. deep. This condition exposed the employee to struck-by hazards.
