This violation was observed on February 8, 2013, in the small pack production area; where at least two (2) of the four (4) locks that were applied to the electric and pneumatic disconnects by employees, who were sanitizing the screw conveyor, did not identify the employee who had applied the locks. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1910.147(c)(5)(ii)(D), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 462779, CITATION NUMBER 001, ITEM NUMBER 005, ISSUED ON NOVEMBER 28, 2012 AND WAS AFFIRMED FINAL ORDER ON DECEMBER 19, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 6000 DENTON DRIVE, DALLAS, TEXAS 75235.
