Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: On February 11, 2013, employees working on service and workover rig TI-500 were exposed to struck-by hazards in that the rig was not leveled according to the manufacturer's instructions. The rear mechanical screws were not extended so as to rest into the rear jack stands.

Rig #1761: On or about February 11, 2013, and times prior thereto, the employer did not provide the mobile work crew working at the rig location access to toilet facilities in accordance with this regulation. This condition exposed employees to potential health hazards.
