OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.      (a) Break room:  On or about 12 February 2013 and times prior thereto, a 50 foot green extension cord was missing the ground pin.  This condition exposed employees to a 110 volt electrical shock and burn hazard.      (b)  Forward Engineering Room:  On or about 12 February 2013 and times prior thereto, the forepeak lighting panel # LP103 was missing three blanks.  This condition exposed employees to a 208/120 volt electrical shock and burn hazard.

29 CFR 1910.23(a)(7):     Every temporary floor opening shall have standard railings, or shall be constantly attended by someone.    (a) Hauling Station, Port side:  On or about 12 February 2013 and times prior thereto, no midrail was installed on the railing adjacent to the open main hatch.  This condition exposed employees to a fall hazard of approximately 11 feet to the deck below.      (b)  Fish Processing Area, Reefer Access hatch:  On or about 12 February 2013 and times prior thereto, The reefer hatch was open with no guardrails or restraints installed.  This condition exposed employees to a approximately 8 foot-7 inch fall hazard to the Reefer room below.      The F/V Siberian Sea was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307507392, Citation 01, Item Number 01, issued 03/02/2011, with respect to a workplace located on the F/V Siberian Sea.
