29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use    Coastal Dock, F/V Siberian Sea:  On or about 07 February 2018 and times prior thereto, the employer had not implemented a written respiratory program for employees who wore tight fitting respirator.  Employees had not been fit tested or had a medical evaluation. This exposed employees to respiratory hazards from improper use.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  Coastal Dock, F/V Siberian Sea, Factory:  On or about 07 February 2018 and times prior thereto, employees were using corrosive chemicals without an adequate eyewash in the area.  Eye wash station had garbage in the wash basin, Monthly inspection had not been accomplished and water tested rusty.  This exposed employees to eye injuries from not having suitable flushing capabilities for immediate use.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  Coastal Dock, F/V Siberian Sea, Reefer Room:  On or about 07 February 2018 and times prior thereto, an air nozzle used by employees did not have 30 P.S.I reducing devices.  Shop air tested was over 100 p.s.i.   This exposed employees to air embolism/laceration hazards if they contacted the discharge to their skin
