(a) Left Side of Building - On or about February 14, 2013 one employee was exposed to a struck by hazard while walking directly under the forks of a Caterpillar 289C Compact Track Loader (serial no. HR2971).

29 CFR 1926.602(c)(1)(viii)(A): Use of a safety platform firmly secured to the lifting carriage and/or forks. (a) Left Side of Building - On or about February 14, 2013 one employee was exposed to a fall hazard while standing on a pallet placed on the forks of a Caterpillar 289C Compact Track Loader (serial no. HR2971). The pallet was not secured to the lifting carriage or the forks. Employee was elevated 8 feet above the ground.
