On or about February 12, 2013, and times prior, employees were exposed to hazardous chemicals at the mud house and no eyewash or drenching facilities were provided. This condition exposed employees to safety hazards.

29 CFR 1910.157(c)(4): The employer did not assure that portable fire extinguishers are maintained in a fully charged and operable condition and kept in their designated places at all times except during use. On or about February 12, 2013, and times prior, a portable carbon dioxide fire extinguisher located in the generator house was not charged and ready for use. This condition exposed employees to potential fire hazards. Bison Drilling & Field Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.157(c)(4) which was contained in OSHA inspection number 315714659, citation number 2, item 1, and was affirmed as a final order on 07-26-2012, with respect to a workplace located at 8.8 Miles NW of Midland, TX 79707.
