29 CFR 1910.304(g)(5): The paths to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective: On or about February 13, 2013, the Mud Logger trailer was not grounded effectively. The ground rod (electrode) was inserted into the soil 11.5-inches. This condition exposed employees to potential electrical shock hazards.
