29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):(a) Spooling trailer on the well site: On or about February 13, 2013, and times prior to, an employee operated the electric cable spooling trailer to dispense electric cable into the well bore. The spooling trailer drive chain and sprockets were not adequately guarded exposing the employee to the hazard of in-running nip points.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: Well site: on or about February 13, 2013, employees supporting the installation of a down hole electric pump were not wearing protective eyewear. This exposed them to the hazard of being struck in the eyes by work generated debris.
