General-duty citation text
29 CFR 1915.89(e)(2): The employer shall ensure that the machinery, equipment, or system is shut down according to the written procedures the employer established. Processing Room: on or about 13 February 2013 and times prior thereto, Baader Machine 607 was isolated on the Baader Distribution Panel P-9. Employer's written procedures required an additional tag placed on the machine to identify energy isolation. This condition exposed employees to caught in and amputation hazards in the event of miscommunication.
