29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about 2/20/13, at the above addressed jobsite, employees working on top of an overhang roof of a commercial property were applying caulking on window frames and were not protected from struck-by hazards.    Instance 1: Two employees working underneath an aerial lift were not protected from overhead struck-by hazards.  Instance 2: An employee working from an aerial lift was not protected from overhead struck-by hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:     On or about 2/20/13, at the above addressed jobsite, an aerial lift operator was applying caulking to the top window of a 2nd level commercial property and did not attach the lanyard to the harness.
