29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a. Wire Stripping Area - On or about Feb 20, 2013, the Mah Tech #15 and #20 wire strippers were inadequately guarded to prevent employees' fingers from entering the point of operation. The employees were thereby exposed to the hazard associated with unguarded points of operation.
