OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:     Well site:  On or about February 14, 2013, and times prior to, employees worked on a drilling rig derrick mounted platform during the drilling of an oil well. The ground anchor stake for the derrick emergency escape system (the Geronimo) was inadequate and prohibited by the manufacturer instructions.  This exposed the employees to the hazard of falling during an emergency egress from the derrick.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    The drilling rig mud system:  On or about February 14, 2013, and times prior to, employees operating the drilling rig mud system during the drilling of an oil well worked with and around high pressure mud hose connections that did not have hose restrains installed.  These employees were exposed to the hazard of being struck by hose end in the event that the hose connection failed in the following instances:    a)  Mud pump #2 discharge hose connecting it to mud pump #1 discharge manifold, the hose connection on mud pump #2 side.    b)  The main mud pump hose running from mud pump #1 to the rig base pipe, connection to the pipe.    c)  The rig base pipe connection to the hose running up to the Kelly stand pipe.

The driller's side hoist line was crushed at the hoist drum.  This exposed the employees to the hazard of being struck by falling loads.

29 CFR 1910.176(b): Storage of material created a hazard:     Drilling rig front yard:  On or about February 14, 2013, and times prior to, employees operating a drilling rig during the drilling of an oil well worked with and around drilling pipe storage rack that did not have the means to prevent the pipe from rolling off the ends of the racks. This exposed the employees to being struck on the lower leg and/or feet by a falling section of pipe.  The racks included:     a)  The rack set along the north side of the catwalk.     b)  The rack set in the northeast corner of the well site.

Alternator belts and pulleys were not adequately guarded exposing the employees to the hazard of being caught in in-running nip points.  The engines involved were:     a)  The #2 generator engine in the light house.     b)   The driller's side and off driller's side drilling engines on the rig carrier.

This exposed the employees to the hazard of electrical shock.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Drilling rig:  On or about February 14, 2013, and times prior to, employees operating a drilling rig during the drilling of an oilwell worked with and around damaged flexible cord assemblies.  This exposed them to the hazard of electrical shock in the following instances:    a)  Mud pump number 2, a cord attached to a dual receptacle.    b)  Mud house, the exhaust fan cord.    c)  Mud house, the cord set running from the mud house to mud pump number 2.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  The rig back yard:  On or about Feburary 14, 2013, employees operating a drilling rig during the drilling of an oil well used and worked around the diesel/bulk lubricants skid.  The skid lubricant tanks were not marked with the contents health hazard information, and the diesel tank was marked as to its contents and health hazards.
