29 CFR 1910.134(c)(2): The employer did not develop and implement a respiratory protection program in accordance with sections i and ii of this paragraph (c)(2) for employees who chose to wear respirators where respirator use was not required:  (a) Anderson Engineering at Wisconsin Rapids Grain facility, Blair WI; The employer did not implement the elements of a written respiratory protection program for employees who performed grain handling and use half face elastomeric respirators, such as, but not limited to:  (1) Provide the employees with the information contained in 29 CFR 1910.134 appendix D, and  (2) A medical evaluation to ensure the employee is able to use a respirator, and   (3) The respirator is clean and stored in a manner that does not present a health hazard.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemcial action or overheating:  (a) Anderson Engineering at Wisconsin Rapids Grain facility, Blair WI; Employees were exposed to open electrical hazards from energized broken light fixtures with missing protective covers and missing light bulbs that were located approximately 18 inches off the ground level in the scale house.

29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on energized circuits and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools:  (a) Anderson Engineering at Wisconsin Rapids Grain facility, Blair WI; Employees were not trained in NFPA 70E and entered an energized electrical cabinet located in the electrical room to turn grain streaming equipment on and off and perform lockout procedures.
