29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.   Throughout Facility - On or about February 21, 2013, the employer did not perform periodic inspections of the energy control procedures, thereby exposing employees to the hazards associated with unexpected re-energization of equipment.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  Throughout facility - On or about February 21, 2013, the employer did not provide effective training to newly hired employees that were authorized, affected, and other employees to include all of the elements in (a) through (c), thereby exposing employees to hazards associated with unexpected re-energization of equipment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. Pipe Shop- On or about February 21, 2013, the employer failed to guard the Wellsaw horizontal band saw, serial # 12904, at the unused portion of the blade, thereby exposing employees to amputation type hazards.    b. Boiler Plant - On or about February 21, 2013, the employer failed to guard the Jet horizontal band saw, serial # WJ 0059, at the unused portion of the blade, thereby exposing employees to amputation type hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a.  Carpentry Department - On or about February 21, 2013, the employer failed to guard the Northfield Foundry and Machine Company vertical band saw, serial # 581554-1, at the point of operation, thereby exposing employees to amputation  type hazards.  b.  Carpentry Department - On or about February 21, 2013, the employer failed to guard the Delta vertical band saw, serial # 89L03169, at the point of operation, thereby exposing employees to amputation type hazards.   The VA Hines was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number313939928, citation number 1, item number 3 and was affirmed as a final order on December 22, 2010, with respect to a workplace located at 5000 South 5th Avenue, Hines, IL 60141.
