29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  The Leadwell MCV-1500i (Vertical Machining Center) had doors which could be opened during operation and the loading/unloading of the parts which was not properly guarded exposing employees to moving parts.  (b)  The Kitamura mycenter 5 (Vertical Machining Center) had doors which could be opened during operation and the loading/unloading of the parts which was not properly guarded exposing employees to moving parts.  (c)  The Leadwell MV-50i (Vertical Machining Center) had doors which could be opened during operation and the loading/unloading of the parts which was not properly guarded exposing employees to moving parts.  (d)  The Haas 20 HP Vector Drive CNC had doors which could be opened during operation and the loading/unloading of the parts which was not properly guarded exposing employees to moving parts.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   (a)  On or about February 11, 2013, the Aida 250 had inadequate guarding when two operators were working on the same piece of equipment exposing one employee to the point of operation.   (b)  The Aida 250 had inadequate point of operation guarding on the back side of the punch press exposing employees to struck by hazards.   (c)  The Aida 200 #2 had inadequate point of operation guarding on the back side of the punch press exposing employees to struck by hazards.   (d)  The Aida 200 #5 had inadequate point of operation guarding on the back side of the punch press exposing employees to struck by hazards.   (e)  The Cincinnati 135 Angled press brake had inadequate point of operation guarding on the back side of the punch press exposing employees to struck by hazards.
