Main Street, Cartersville, Ga., An employee was exposed to  falls of approximately 13 feet to the ground below while working on scaffold platforms which were not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a) 459 E. Main Street, Cartersville, Ga., On the West side of a commercial building (BoJangles Restaurant) An employee was exposed to fall hazards of approximately 13 feet while climbing tubular welded scaffold frames and cross bracing when a ladder or other safe means of access was not provided to access the scaffold work platform.

Main Street, Cartersville, Ga., On the West and South of a commerical buidlding, an employee was exposed to falls of approximately 13 feet while applying stucco on the exterior of BoJangles Restaurant, working from a tubular welded scaffold which was not provided with guardrails.
