29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports:      On or about February 21, 2013, at the work site, an employee was cutting metal supports in order to lower the front of the roofs facial from a scaffolds 2nd tier that had only three of approximately five planks exposing the employee to a fall of approximately 12 feet high to the concrete ground level below.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    On or about February 21, 2013, at the work site, an employee working on a scaffold's 2nd level that had missing base plates or footing and was cutting metal supports in order to lower the front of the facial and was exposed to a fall of approximately 12 feet high to the concrete ground level below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about February 21, 2013, at the work site, an employee working on a scaffold's 2nd level platform was cutting metal supports in order to lower the front of the facial and was exposed to a fall of approximately 12 feet high to the concrete ground level below while climbing the supports and crossbracings.
