29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about 4/2/13 - 501 15th Street East, Tuscaloosa AL, employees installing metal framing for a roof were exposed to a fall hazard of 14 feet without protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about 2/22/13, 501 15th Street East, Tuscaloosa AL, a portable ladder on the south side of the building used to access the top of a building under construction was not extended 36 inches above the structure's edge.
