29 CFR 1910.23(a)(8):   Every floor hole into which persons can accidentally walk was not guarded:    a)  At the north end of the equipment storage aisle adjacent to the kitchen, on or about February 22, 2013 and at times prior thereto, employees were exposed to a tripping hazard from an open four-inch drain hole, while carrying utensils in the walkway.    b)  At the south end of the equipment storage aisle adjacent to the kitchen, on or about February 22, 2013 and at times prior thereto, employees were exposed to a tripping hazard from an open four-inch drain hole, while carrying utensils in the walkway.

29 CFR 1910.151(c):  Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    In the kitchen, on or about February 22, 2013, and at times prior thereto, employees were exposed to permanent eye injury, from the lack of adequate emergency eyewashing equipment, while using Ken's Essentials Disinfectant Ultra Bleach to sanitize countertops and food preparation areas.

29 CFR 1910.212(a)(1):   One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Near the center of the kitchen floor, on February 22, 2013, employees were exposed to an amputation hazard while operating the Hobart vertical mixer, Model D-3001, Serial 11-1011-285,  30-quart, without a guard around the rotating parts.
