29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) D-7 Roofing LLC at 928 W 14th Ave, Denver, CO 80204: On February 5, 2013 the exposing employer did not ensure that employees were protected from injury, in that five employees performing residential roofing work on a one-story, multi-unit complex were not utilizing an adequate fall protection system. This condition exposed employees to a fall hazard of approximately 13 feet. D-7 Roofing LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 556240, Citation 2, Item 1, issued on November 16, 2012 and became final order on January 2, 2013 with respect to a workplace located at 14049 St. Paul Street, Thornton, CO 80602.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) D-7 Roofing LLC at 928 W 14th Ave, Denver, CO 80204: On February 5, 2013 the exposing the employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection, in that five employees performing residential roofing work on a one-story, multi-unit complex were not utilizing an adequate fall protection system. This condition exposed employees to a fall hazard of approximately 13 feet. D-7 Roofing LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.503(a)(1) which was contained in OSHA Inspection Number 314660689, Citation 1, Item 1b, issued on November 10, 2010 and became final order on January 3, 2011 with respect to a workplace located at 10154 Ouray St, Commerce City, CO 80022.
