29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    On or about February 21, 2013 at the paint and warehouse areas;    The stairways, platforms, and storage area platform used by the employees to access the storage areas above the paint shop and warehouse area did not have a mid-rail installed to protect the employees from hazards up to 12 feet from ground level in the following locations:      1). Main warehouse two stairways were missing midrail and exposed the employees to a fall hazard of 12 feet .    2). The stairway platform in the paint shop leading to the upper storage area was missing the midrail and exposed the employees to a fall hazard of 10 feet .    3). The upper storage area in the paint shop was missing a section of the midrail, which exposed the employees to a fall hazard of 10 feet 6 inch .

In the refurbish shop area there were several pressurized and partially filled compressed gas cylinders, that are used with the torch set to cut and weld metal material in which the valve protection caps were missing.
