OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed as a result of attempting to by-pass the safety interlock on the cardboard compactor. On or about 01/30/2013, at Dock Door 100, the Cram-A-Lot Cardboard Compactor, unknown m/n or s/n, was being operated by an employee with the gate in the open position. The magnetic activated interlock switch had been by-passed and allowed the ram to cycle with the gate open resulting in injuries to the employee. One method of abatement to correct this apparent hazard is to comply with ANSI Z245.2, paragraph 5.8.1. This standard requires that access door(s) of loading chambers shall have an interlock system that prevents cycling motion while the access door(s) is open. Paragraph 5.8.2 states that an interlock shall not be readily defeated without intentional purpose, such as by: a) Damaging of the interlock, b) Making wiring connections or alterations; or c)The use of tools or other materials (such as magnets and wedging devices). DISCLAIMER: a) The employer is not limited to the abatement method(s) suggested by OSHA b) The employer is responsible for selecting and implementing an effective abatement method.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about 02/06/2013 in an aisle approaching Dock 6512, the mixed states area, a forklift used by the operator to bulldoze four pallets of letter trays made contact with a fellow forklift operator, resulting in injuries to the employee.
