29 CFR 1926.451(e)(8):  Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:  (a) On or about 02/15/13 - At 5834 Cahaba Valley Road, Birmingham, AL 35242, employees accessing the JLG scissor lift scaffold from the roof climbed over the railing platform.

29 CFR 1926.760(a)(1):  Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  (a) On or about 02/15/13 - at 5834 Cahaba Valley Road, Birmingham, AL  35242, employees working at an unprotected edge while installing a metal roof approximately 20 to 27.6 feet above the lower lever were not protected from falling.
