29 CFR 1926.651(c)(2):     A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:     a) Excavation located north of Ironwood and Wood Creek Billings, MT 59106: On February 25, 2013 and at times prior there to, employee(s) were exposed to the hazards of not being able to safely egress excavations approximately 12 feet deep while using an in house fabricated ladder attached to the cross members of an excavation.

29 CFR 1926.651(k)(2):     Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety.    a)  Excavation located north of Ironwood and Wood Creek Billings, MT 59106:  On February 25, 2013, and at times prior thereto, employees were exposed to the hazard collapse due to an imporperly positioned trench box and failure to provide adequate access and egrees for employee(s) installing a sewer line.

29 CFR 1926.652(g)(2):     Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted:    a) Excavation located north of Ironwood and Wood Creek Billings, MT 59106: On February 25, 2013 and at times prior there to, employee(s) were exposed to caught in-between and crushing hazards from working in an excavation approximately 12 feet deep without adequate cave in protection. The bottom of the trench shield was approximately 4 feet from the bottom of the excavation.
