( c ) On the pavement and on pallets in the parking area near the Old Hazardous Materials Building 23 on or about March 6, 2013, and at times prior thereto, an endless wire rope sling with a 2 inch diameter did not have circumferential length of 96 times 2 inches or 192 inches as the circumferential length was about 154 inches.     (d) On the pavement and on pallets in the parking area near the Old Hazardous Materials Building 23 on or about March 6, 2013, and at times prior thereto, an endless wire rope sling with a 1.5 inch diameter did not have circumferential length of 96 times 1.5 inches or 144 inches as the circumferential length was about 94 inches.     (e) On the pavement in the parking area near the Old Hazardous Materials Building 23 on or about March 6, 2013, and at times prior thereto, an endless wire rope sling with a 1.75 inch diameter did not have circumferential length of 96 times 1.75 inches or 168 inches (14 feet) as the circumferential length was about 132 inches (11 feet). (d) On the pavement near the east side exterior of the Rigging Shop in Building 21, on or about March 6, 2013, and at times prior thereto, a 1.25 inch diameter by 20 foot long wire rope sling, SN: WR-1.25-0002, was not removed from service when the ends were crushed and the core was visible between the strands at the ends.     (e) In a storage box on the pavement near the east side exterior of the Rigging Shop in Building 21, on or about March 6, 2013, and at times prior thereto, a 1.25 inch diameter by 20 foot long wire rope sling, SN: WR-1.25-0001, was not removed from service when the rope was kinked and the core was visible between the strands about halfway along the length of the sling. (e) In the Machine Shop in Building 20 on or about February 27, 2013, and at times prior thereto, there was no guard to protect employees from the rotating table on a King Stedfast Ralston Machinery, Inc., Boston, MA, vertical turret lathe, Guam Shipyard number 004305. The rotating table was 52 inches in diameter, and the top of the table was 39 inches from the floor. The     (f) In the Machine Shop in Building 20, on or about February 27, 2012, and at times prior thereto, there was no guard to protect employees from the chuck on a vertical drill press, Guam Shipyard number 001290.    (g) In the Machine Shop in Building 20, on or about February 27, 2012, and at times prior thereto, there was no guard to protect employees from propeller shafts that rotated in an American Tool Works Co., Cincinnati, USA turret, Guam Shipyard number 000159.    (h) In the Machine Shop in Building 20, on or about February 27, 2012, and at times prior thereto, there was no guard to protect employees from the rotating chuck with protruding jaws on a Monarch turret lathe, Guam Shipyard number 004292. The lathe was used on flanges, coupling and motor rotors. The foreman and leader for the Machine Shop said the machinists used the machine about two times per month.    (i) In the Pipe-fitters Shop in Building 21, on or about March 1, 2013, and at times prior thereto, there was no guard to protect employees from the chuck on a Morris Mor-Speed radial drill, Guam Shipyard number 004307. The shipyard serviced multiple cranes, benders, shears, ironworkers, presses, sanders, saws and press brakes in their shops. The program and procedures were required to be recorded and maintained by the employer under 29 CFR 1915.89(r)(1). The program and procedures include:     - Procedures for lockout/tags-plus systems while servicing machinery, equipment, or systems in accordance with paragraph (c) of this section;    - Procedures for protecting employees involved in servicing any machinery, equipment, or system in accordance with paragraphs (d) through (m) of this section;     - Specifications for locks and tags-plus hardware in accordance with paragraph (n) of this section;     - Employee information and training in accordance with paragraph (o) of this section;     - Incident investigations in accordance with paragraph (p) of this section; and     - Program audits in accordance with paragraph (q) of this section.

29 CFR 1915.89(c)(1): The employer did not ensure that, before any authorized employee performs servicing when energization or startup, or the release of hazardous energy, may occur, all energy sources were identified and isolated, and the machinery, equipment, or system was rendered inoperative:     (a) In the Building 20 Machine Shop on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative on Rockford Machine Tool Co. slotter, Guam Shipyard number 001619, when the cutter die was changed or when the V-belts were changed on an electric motor located on the side of the machine. A control button for one of the motors was located on the side of the machine. The cutter die was used to put key-ways on metal parts, and the die was changed when the job changed. A foreman and a leader for the Machine Shop said that the machinists used the machine about three times per year and changed the die at least each time it was used. A foreman for the Machine Shop said the guard for the belts was removed when the belts were replaced a few months ago.    (b) In the Building 20 Machine Shop on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative on a DoAll horizontal, continuous blade band saw, Guam Shipyard number 001837, when the saw blade was changed. A machinist can be located on the opposite side of the control buttons while changing the saw blade, making the control button inaccessible. A foreman and a leader for the Machine Shop said that the machinists changed the blade about three times per year.    ( c ) In the Building 20 Machine Shop on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative on a Kasto Racine, Inc. power hack saw, Guam Shipyard number 003880, when the saw blade was changed. A machinist can be located on the opposite side of the control buttons while changing the saw blade, making the control button inaccessible. A foreman and a leader for the Machine Shop said that the machinists changed the blade about three times per year.    (d) In the Building 20 Machine Shop on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative on a Adwalt Products abrasive wheel cut off saw when the abrasive wheel or belts were changed. A foreman and a leader for the Machine Shop said that the machinists changed the wheel and belts about once per year.    (e) In the Building 20 Machine Shop on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a 60 foot long American Tool Works Cincinnati USA horizontal turret, Guam Shipyard number 000159. The Machine Shop foreman said machinists clean the turret about four times per year. The Machine Shop foreman said machinists setup the propeller shaft, from the US Coast Guard Cutter Assateague, in the turret about two weeks ago to check the straightness of the shaft. Employees can be located up to 60 feet from the turret's control buttons.     (f) In the Building 20 Machine Shop on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a 100 foot long Monarch turret lathe, Guam Shipyard number 004292. The Machine Shop foreman said machinists clean the turret about four times per year. The turret was used to align ships' propeller shafts. Employees can be located up to 100 feet from the turret's control buttons.     (g) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior there to, the electrical energy was not identified, isolated and rendered inoperative before servicing a Diehl Woodworking Machine line saw. A shipwright foreman said the wood crafters changed the blade about four times per year.    (h) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior there to, the electrical energy was not identified, isolated and rendered inoperative before servicing a Newman Machine Co. single surface planner. A shipwright foreman said the wood crafters changed the blade about one time per year.    (i) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Wysong Sander, Guam Shipyard number 002457. The shipwright foreman said the wood crafters changed the sand paper on the rotating disc and shaft about one time per month.    (j) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Tannewitz 0.5 inch band saw, Guam Shipyard number 000723. The shipwright foreman said the wood crafters changed the blade about one time every two months.     (k) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Delta table saw. The shipwright foreman said the wood crafters changed the oil and cleaned the machine about six times per year.    (l) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Buss double surface planer, The shipwright foreman said the wood crafters changed the oil and cleaned the machine about two times per year.    (m) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a DeWalt radial arm saw, Guam Shipyard number 000298. The shipwright foreman said the wood crafters changed the blade about two times per year when it started to wobble.    (n) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Tannewitz band saw, Guam Shipyard number 004096. The shipwright foreman said the wood crafters changed the blade and cleaned about two to three times per year.    (o) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing two DeWalt radial arm saws, Guam Shipyard numbers 000186 and 000187. The shipwright foreman said the wood crafters maintained the blades about two times per year.    (p) In the Building 21 Insulation Shop on or about February 28, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Northfield Foundry & Machine Co. table saw, Guam Shipyard number 002243. The shipwright foreman said the wood crafters checked the blade about one time per month.    (q) In the Building 21 Ship-fitters Shop, on or about February 28, 2013, and at times prior thereto, the electrical energy and hydraulic energy were not identified, isolated and rendered inoperative before servicing a 300 ton Erie hydraulic press, Guam Shipyard number 00064. The maintenance leader said the maintenance staff maintained the equipment about two times per year.    ( r ) In the Building 21 Ship-fitters Shop, on or about February 28, 2013, and at times prior thereto, the electrical energy, pneumatic energy and hydraulic energy were not identified, isolated and rendered inoperative before servicing a Cleveland Crane & Engineering shear, serial number 8985, model number 16F-12. The shear was used to cut one inch thick mild steel. The maintenance leader said the maintenance staff maintained the equipment about two times per year.    (s) In the Building 21 Ship-fitters Shop, on or about February 28, 2013, and at times prior thereto, the electrical energy and hydraulic energy were not identified, isolated and rendered inoperative before servicing a Chicago Dreis & Krump bender or press brake, Guam Shipyard number 001991. The maintenance leader said the maintenance staff cleaned and repaired leaks on the equipment about two times per year.    (t) In the Building 21 Ship-fitters Shop, on or about February 28, 2013, and at times prior thereto, the electrical energy and mechanical energy were not identified, isolated and rendered inoperative before servicing a Buffalo Universal Iron Worker, order number 70W3004. The maintenance leader said the maintenance staff cleaned and repaired leaks on the equipment about two times per year.    (u) In the Building 21 Ship-fitters Shop, on or about February 28, 2013, and at times prior thereto, the electrical energy and mechanical energy were not identified, isolated and rendered inoperative before servicing a Buffalo Universal Iron Worker, order number 66W-6358. The maintenance leader said the maintenance staff cleaned and repaired leaks on the equipment about two times per year.    (v) In the Building 21 Pipe-fitters Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Do-Rite Machinery Service, Inc. pipe bender, Guam Shipyard number 004435. The pipe fitters foreman said the pipe fitters changed the pipe die about one time per month.    (w) In the Building 21 Pipe-fitters Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Wallace Supplies Manufacturing Co. pipe bender, Guam Shipyard number 414349. The pipe fitters foreman said the pipe fitters changed the pipe die about one time per month.    (x) In the Building 21 Pipe-fitters Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy and hydraulic energy were not identified, isolated and rendered inoperative before servicing a Hannafin Corporation hydraulic press, Guam Shipyard number 001674. The press was used to flatten steel. The pipe fitters foreman said the maintenance staff cleaned and repair leaks on the equipment about one time per month.    (y) In the Building 21 Sheet Metal Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Kalamazoo metal cutting band saw, Guam Shipyard number 004394. The band saw was used to cut metal pipe. The pipe fitters foreman said the pipe fitters changed the blade on the equipment about two times per year.    (z) In the Building 21 Sheet Metal Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy, hydraulic energy and mechanical energy were not identified, isolated and rendered inoperative before servicing a Universal Iron Worker, Guam Shipyard number 004381. The iron worker was used to punch holes in sheet metal. The sheet metal leader said the sheet metal staff changed the dies to punch different size holes in the sheet metal about one time per week.    (aa) In the Building 21 Sheet Metal Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy, and hydraulic energy were not identified, isolated and rendered inoperative before servicing a Cincinnati shear, Guam Shipyard number 005800. The shear was used to cut sheet metal, When the shear starts to bend the sheet metal instead of cutting it, a mechanic must adjust the cutting blade. The sheet metal leader said the mechanic must adjust the blades about two times per year.     (bb) In the Building 21 Sheet Metal Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before servicing a Niagara power roller, Guam Shipyard number 002608. The roller was used to form sheet metal. The sheet metal leader said the mechanics must adjust the gears about two times per year.    (cc) In the Building 21 Sheet Metal Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy and hydraulic energy were not identified, isolated and rendered inoperative before servicing a Pacific Hydraulic Press Brake, Guam Shipyard number 002276. The press brake was last used to form 3/16s inch sheet metal. The sheet metal leader said the press brake was last used in November 2012 and sheet metal staff changes the blades for different bends about two times per year. The sheet-metal leader also said a co-worker was nearly struck by a bender when changing the dies in the Philippines.    (dd) In the Building 21 Sheet Metal Shop, on or about March 1, 2013, and at times prior thereto, the electrical energy and hydraulic energy were not identified, isolated and rendered inoperative before servicing a Pullman Universal Shearing and Forming Machine, Guam Shipyard number 042855. The machine was used to make and cut gaskets out of a plastic type of material. The sheet metal leader said the dies were changed to cut metal and the sheet metal staff changes the dies about one time every three years.    (ee) In Building 20, on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before performing inspections on the following jib or wall mounted electrical cranes manufactured by Coffing with rated load capacities from 500 to 4000 pounds: J-1 through J-9, J-14, J-18 through J-20, J-22, J-36, J-38, J-40 through J-54, and J-68. Inspections on the hooks and running ropes are potentially required monthly.    (ff) In Building 21, on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before performing inspections on the following jib or wall mounted electrical cranes manufactured by Coffing with rated load capacities from 1000 to 4000 pounds: J-21, and J-58 through J-61. Inspections on the hooks and running ropes are potentially required monthly.    (gg) In Building 20, on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before performing inspections on the following monorail cranes: M-5 Coffing 5000 pound, M-31 CM 1000 pound, and M-57 Coffing 1000 pound. Inspections on the hooks and running ropes are potentially required monthly.    (hh) In Building 21, on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before performing inspections on the following monorail cranes: M-12 Dresser 4000 pound, M-6 Coffing 1000 pound, and M-30 Dresser 1000 pound in the rigging shop. Inspections on the hooks and running ropes are potentially required monthly.    (ii) In Building 20, on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before performing inspections on the following bridge cranes: B-1/36 NESCO 20 ton/5 ton, B2/20681 Harnischfeger Corp. 5 ton, B-3 P&H 3 ton, B-4/20686 Harnischfeger Corp. 5 ton, and B-6/20687 Harnischfeger Corp. 10 ton. Inspections on the hooks and running ropes are potentially required monthly.    (jj) In Building 21, on or about February 27, 2013, and at times prior thereto, the electrical energy was not identified, isolated and rendered inoperative before performing inspections on the following bridge cranes: B-7/20684 Harnischfeger Corp. 5 ton, B-8/20682 Harnischfeger Corp. 5 ton, B-9/20683 Harnischfeger Corp. 5 ton, B-10/20685 Harnischfeger Corp. 5 ton, and B-15/22785 Harnischfeger Corp. Inspections on the hooks and running ropes are potentially required monthly.    (kk) In Building 20 in the Machine Shop, on or about March 5, 2013, the possible energy generated by a parked, flatbed trailer was not adequately identified, isolated and rendered inoperative before a 18,000 pound engine from the US Coast Guard Cutter Assateague was lowered onto the flatbed trailer with a NESCO 20ton/5ton bridge crane, B-1/36. There were no brakes or wheel chocks to secure a flatbed trailer from moving when riggers were on the trailer and next to the trailer while the engine was lowered onto the trailer with the bridge crane. The only means used to secure the trailer from moving was a steel encased support secured to the trailer's kingpin. The flatbed trailer was about 33 feet long, had two axles with dual wheels (8 wheels), weighed 40,000 pounds, Reg. # 97-37083, and a date of delivery of 2/88. The brake chambers had been removed from the trailer. (f) In Building 21 in the Welders Shop on or about March 6, 2013, and at times prior thereto, a Renfroe grab clamp, model TLA, was not removed from service when the markings that identify its capacity were not legible. The grab clamp, a 3 foot long, 0.875 inch diameter wire rope sling, and the 5 ton Harnischfeger Corporation bridge crane, B-8/20682, were used to lift a 20 long, 700 pound T bar.    THE GUAM INDUSTRIAL SERVICES DBA GUAM SHIPYARD WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1915.111(A) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315278036, CITATION NUMBER 1, ITEM NUMBER 15, AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 19, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT THE GUAM SHIPYARD, SANTA RITA, GUAM.
