29 CFR 1915.72(a)(3): Portable ladders shall be lashed, blocked or otherwise secured to prevent their being displaced. On or about 02/21/2013, the employer did not ensure that ladders used by employees while on docked work boats were secured in place to prevent tipping or other type of displacement. Employees did not maintain 3 point contact with the ladder while climbing/desending the 6 ft step ladder while carrying the radar dome cover, which could cause the ladder to tip, allowing the employee to fall a distance of 4 to 30 ft.

29 CFR 1915.159(d): Training. Before using personal fall arrest equipment, each affected employee shall be trained to understand the application limits of the equipment and proper hook-up, anchoring, and tie-off techniques. Affected employees shall also be trained so that they can demonstrate the proper use, inspection, and storage of their equipment. [61 FR 26321, May 24, 1996; 67 FR 44544, July 3, 2002] On or about 02/21/2013, the employer did not enure that employees that have the possibility of using Personal Fall Arrest Systems (PFAS) in conjunction with the performance of their work were trained in the limitations of the PFAS, hook-up and anchorage techniques and the proper use, inspection and storage iof the PFAS assigned to them. The employer provides PFAS to the employees, but did not train them in the proper use, inspection and storage of the PFAS.
