29 CFR 1926.416(a)(1): Employer permitted employees to work in proximity to electric power circuits without the employees being protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. a) "3rd Base" Switchgear Room, PNC Stadium, 235 Montage Mountain Road, Moosic, PA: An employee attaching conductors to the DP-MC-C2-1 circuit breaker located in the "3rd Base" switchgear received an electric shock. The electric power circuit was energized by the "1st Base" switchgear that was "backfeeding" the circuit through the DP-MC-C2-1 panel, on or about February 21, 2013. In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.

29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits could be energized. a) "3rd Base" Switchgear Room, PNC Stadium, 235 Montage Mountain Road, Moosic, PA: The DP-MC-C2-1 Electrical Panel "Main" circuit breaker was deenergized but was not rendered inoperative or tagged. Persons unknown reenergized the DP-MC-C2-1 Electrical Panel "Main" circuit breaker and an employee attaching conductors from the DP-MC-C2-1 Electrical Panel to the DP-MC-C2-1 circuit breaker located in the "3rd Base" switchgear received an electric shock, on or about February 21, 2013. In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.
