29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. On or about 03/06/2013, at the work table in the center of the shop the employer failed to keep the employees safe from electrical shock by properly covering the outlet box on the grinding table.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. On or about 03/20/2013, the employer failed to keep the portable fire extinguishers easily and readily accessible to employees. The extinguishers could not be properly hung in place due to other equipment hanging where the fire extinguishers should be.
