29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  (a). 4755, I-55 N., Jackson MS - On or about February 26, 2013 employees were working from a welded frame scaffold that did not have fully planked platforms.  The scaffold was three tiers high.

29 CFR 1926.451(g)(1):   Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.   (a). 4755, I-55 N., Jackson MS - On or about February 26, 2013, employees were working from a 19 feet high welded frame scaffold and did not have guardrails or other forms of fall protection to prevent them from falling off the scaffold and striking the ground.

29 CFR 1926.451(f)(15):   Ladders shall not be used on scaffolds to increase the working level height of employees, except on large area scaffolds.  (a). 4755, I-55 N., Jackson MS - On or about February 26, 2013, an employee was standing on a step ladder that was placed on the top platform of a three tier scaffold. The step ladder was leaned against the building; the employee was using the ladder to gain additional height.

29 CFR 1926.405(j)(1)(i):   Live parts. Fixtures, lampholders, lamps, rosettes, and receptacles shall have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts.  (a). 4755, I-55 N., Jackson MS - On or about February 26, 2013, a quadruplex outlet receptacle box had a missing knock out plug.
