29 CFR 1910.132(d)(1):     The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:    a.  At the work site located at 3660 Parkway Lance Suite A, Hilliard, Ohio, the employer did not have a written hazard assessment of the workplace for employees who performed duties including (but not limited to) mixing and testing inks, adding drying agents to ink blends, using cleaners to remove ink, and propane used by a fork lift truck.    b.  At the work site located at 4477 Reynolds Drive, Hilliard, Ohio, the employer did not have a written hazard assessment of the workplace for employees who performed duties including (but not limited to) mixing and testing inks, adding drying agents to ink blends and using cleaners to remove ink.

At the work site located at 3660 Parkway Lane Suite A, Hilliard, Ohio, employees used mixers to combine inks to customer specifications and add drying agents, and used a blanket wash cleaning solution to remove ink from the equipment, counters, and ink knives.  The employer did not ensure that employees  had eye protection available and kept it in a sanitary condition.  b.  At the work site located at 4770 Reynolds Drive, Hilliard, Ohio, employees used mixers to combine inks to customer specifications and add drying agents, and used a blanket wash cleaning solution to remove ink from the equipment, counters, and ink knives.  The employer did not ensure that employees  had eye protection available and kept it in a sanitary condition.

At the work site located at 3660 Parkway Lane, Suite A, Hilliard, Ohio, employees operated fork lift trucks to unload delivery trucks, move delivered stock in the warehouse, load trucks for delivery of large orders, and for placing/removing large barrels of inks that were mixed on the large floor mixers.  The employer had not provided training for employees who operated the fork lift truck.

Combustible waste material and residues in a building or unit operating area shall be kept to a minimum, stored in covered metal receptacles and disposed of daily.    a.  At the branch work site located at 3660 Parkway Lane Suite A, Hilliard, Ohio, employees used rags wetted with flammable  blanket wash solution to clean equipment, counters, and test printers.  The used rags were placed in an open barrel with no cover and left open until the barrel was picked up by an industrial uniform service.
