29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    a) At the Drill and Rig Division the work shop and bench work maintenance area; power tools laying on floor and, sharp hand tools and fabricated materials laying on the floor and walk areas causing slip, trip, and fall hazards.      b) At the Drill and Rig Division storage floor and adjacent to steel racks employees were exposed to pinch and caught between hazards when accessing stored materials; 5 to 50 gallon containers of oil and lubricants, crowded hang racks of rigging hardware, and body harnesses and lanyards.

29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace.    a) At the Drill and Rig Divison work shop, hanging off the top of an anchored "Victor" manufactured drill press a clear face shield was gouged and tar smudged obscuring vision.    b) At the Drill and Rig Divison work shop bench table, hanging on the personal equipment rack, was a clear face shield with obscured vison because of gouges and oil and tar smudges.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    a) At the USGS Western Drill and Rig Division work shop, one ABC five (5 lb) pound and one ABC two and one half (2 1/2 lb) pound fire extinguishers were sitting on the floor between two portable tool lockers.       b) At the USGS Western Drill and Rig Division workshop, pallet storage rack. Access to one ABC five (5 lb) pound fire extinguisher was blocked by a chair, hand tools in a container and blue bucket sitting on a worktable.        c) At the USGS Western Drill and Rig Division, a (5 lb) pound wall mounted ABC fire extinguisher was blocked from view and inaccessible because of stacked boxes and stored portable heaters on a pallet in front of the fire extinguisher.
