29 CFR 1910.37(a)(3): The employer did not ensure that all exit routes were free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route.     a) Bulk Chemical Storage Area -  A rear exit door was blocked by card board boxes with miscellaneous objects inside the boxes.      In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a) Facility Wide - The Yale fork lift truck (s/n - E187V21354A) was equipped with a front-end attachment (Poly Jaws Fork Mounted Drum Grab), however the employer did not request that the truck be marked to identify the attachment and show the approximate weight of the truck and attachment combination.      In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
