29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:    a)  U.S. Route 46 East, Denville, NJ.    Training on the proper use of step ladder and the hazards associated with their placement and use, was not provided to employee on this jobsite by a competent person.                                           Violation observed on or about 02/25/2013.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a)  U.S. Route 46 East, Denville, NJ.    Employees working low-slope roof laying cement block was not protected from fall the next lower level by guardrali system, safety net system, and personal fall arrest system, thereby exposing the employees to a fall hazard of approximately 15 feet to the ground below.                                                Violation observed on or about 02/25/2013.

29 CFR 1926.1053(b)(1): Where a step ladder was used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    a)  U.S. Route 46 East, Denville, NJ.    Employee egress from tubular frame scaffold step on the top rung of a step ladder that did not go 3 feet above upper landing surface, thereby exposing the employee to a 6 feet fall hazard to the ground.                                         Violation observed on or about 02/25/2013.    Joe Maggio, LLC was previously cited for a violation of this Occupational Safety and Health or its equivalent 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection Number 313753824, Citation Number 2, Item Number 1, with a final order issued on 01/13/2010 with respect  to a workplace located at 5 Bowling Green Parkway, Lake Hopatcong, NJ.    Joe Maggio, LLC was previously cited for a Violation of this Occupational Safety and Health or its equivalent 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection Number 312272420, Citation Number 1, Item Number 1 with a final order issued on 10/22/2008 with respect to a workplace located at 1481 Route 23 Butler, NJ.
