29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.    On or about February 27, 2012, employees were performing decking work on a steep roof without fall protection and safety glasses while other employees worked below and used circular saws to cut plywood without hard hats or safety glasses and did not recognize the hazards associated with these conditions.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.   On or about February 27, 2013, the employer did not ensure employees were provided eye protection. Employees without eye protection were cutting wood with an electric circular saw and exposed to eye injuries.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.   On or about February 27, 2013, the employer did not ensure electrical equipment used by employees was equipped with a ground. Employees using an electric circular saw connected to a flexible electrical cord with the ground removed were exposed to an electrical shock.

Flexible cords connected to devices and fittings did not have strain relief provided to  prevent pull from being directly transmitted to joints or terminal screws.   On or about February 27, 2013, the employer did not ensure circular saws connected to flexible cords did not have strain relief. Employees using the electric circular saws to cut plywood were exposed to electric shock hazards.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On or about February 27, 2013, the employer did not ensure employees performing decking work at a height more than 6 feet were protected from falling. The employees were exposed to an unprotected fall to the ground below.

The employees on the ground were exposed to being struck-by objects falling from a height of over 15 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   On or about February 27, 2012, the employer did not ensure employees exposed to fall hazards were trained to recognize and avoid such hazards. The employees working without fall protection were exposed to an unprotected fall of over 15 feet to the ground below.

On or about February 27, 2012, the employer did not ensure the stairs employees were using to access the roof was equipped with one handrail.
