29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated: On or about 4/26/13, the employer failed to submit abatement documentation for the following citations issued on March 12, 2013; 1. Citation 1, Item 1: 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about 2/26/13, the employer did not ensure that the workers were trained to recognize and avoid unsafe conditions applicable to their work environment. 2. Citation 1, Item 2: 29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about 2/26/13, the employer did not provide fall protection to two workers working on the ladder jack scaffold system and exposed them to fall hazards of on or about 18-feet. 3. Citation 1, Item 3: 29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces: On or about 2/26/13, the employers ladder jack scaffold was placed on an icy surface and the extension ladders feet werre in the upright position and not used. The ladder slipped out from underneath causing one of the two workers on the ladder jack scaffold to fall of up to 18-feet to the icy ground below.
