29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed: (a) NW corner of parking garage under construction at the Vee Quiva Casino and Hotel Project (under construction), 15091 S Komatke Lane, Laveen, AZ 85339: On or about 26 February 2013, the Employer did not ensure that an 8 foot portable step ladder was used as designed when an employee was welding a brace above his shoulder height while facing away from the ladder with one foot on the 5th step of the ladder and his other foot atop a 42 inch guardrail of the landing between the ground level and 2nd level of the parking garage, exposing the employee to fall hazards.
