Grinding Area - On or about March 11, 2013, employees were required to operate a - Jones Superior Machine - band saw without a guard to protect the exposed parts of the rotating saw blade. Employees were thereby exposed to cuts, lacerations, and amputations.

29 CFR 1910.132(f)(1):     The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following:    (i) When PPE is necessary;  (ii) What PPE is necessary;  (iii) How to properly don, doff, adjust, and wear PPE;  (iv) The limitations of the PPE; and  (v) The proper care, maintenance, useful life and disposal of the PPE.    a. Employee Lunch Room/Locker Room/Rest Room - On or about march 11, 2013, the employer did not establish a housekeeping schedule to reduce lead accumulation to the lowest level parcticable.  1. The wipe sampling # 10 from the chair/table/night stand showed accumulation of 38.46 micrograms per 10 square centimeters of area.  2. The wipe sampling # 20 from the chair/table/night stand showed accumulation of 115.0 micrograms per 10 square centimeters of area.  3. The wipe sampling # 30 from the chair showed accumulation of 15.18 micrograms per 10 square centimeters of area.
