29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Rear Right Side Roof of Structure - Employees were observed installing exhaust vents on a low sloped (4-12 slope) roof without any means of fall protection, exposing them to fall hazards of 40 feet, on or about 02/27/13. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. a) Rear Right Side Roof of Structure - The employer did not ensure employees installing exhaust vents and thus exposed to a 40 foot fall were adequately trained in hazard recognition or procedures to minimize fall hazards, on or about 02/27/13.
