29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation: a) On or about February 27, 2013, and at times prior, the employer did not record information on the OSHA 300 log for the 2012 and 2013 calendar years with thedetail required in Column F: 1) 2012 cases WC949A72664 and WC949A80082 were missing the objects/substances that directly injured or made the people ill. 2) 2013 cases WC949A88136 and WC949A88544 were missing the objects/substances that directly injured or made the people ill. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
