29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    (a) Jobsite, South Greengate, Greensburg, PA:  On or about 02/28/13 an employee operating a Makita power saw to cut pieces of 2 x 4 stock was not provided with eye protection to protect him from potential eye injury from flying particles or pieces.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Jobsite, South Greengate, Greensburg, PA:  On or about 02/28/13 employees working on the third level of the structure, near and at the edge, lying floor sheathing approximately 19 feet above the ground, were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems.    OR IN THE ALTERNATIVE:    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) Jobsite, South Greengate, Greensburg, PA:  On or about 02/28/13 employees working on the third level of the structure, near and at the edge, lying plywood floor sheathing approximately 19 feet above the ground, were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    (a) Jobsite, South Greengate, Greensburg, PA:  On or about 02/28/13 several pieces of reinforcing steel were not guarded to eliminate the hazard of impalement.  Employees were working above this area, at an unprotected edge, and were exposed to potential impalement hazards if a fall should occur.
